The systematic evaluation of the musculoskeletal system is demonstrated in a young man with acute polyarthritis. The differential diagnosis and treatment of rheumatoid arthritis is then discussed.

First, the systematic demonstration of the shoulder, elbow and wrist joints is given on a normal young man, along with the relevant surface anatomy. Then the examination of the hand in a patient with rheumatoid arthritis is demonstrated with particular emphasis on composite function.

First, gait is discussed in general, and the origin of a limp. The systematic examination of the hips is then demonstrated on a man with severe osteoarthritis of both hips, followed by the examination of a normal subject.

The problem-orientated approach to the examination of the knee is demonstrated in two different subjects with different problems and concerns. First, a younger man with pain, swelling and locking of the knee, (with emphasis on the underlying anatomy);and second in an elderly lady with painful osteoarthritis. Finally, the examination of the ankle is undertaken in a woman with a previous fracture of the ankle and pain on walking.

Paget's Disease of Bone is an extremely common focal bone disease in some parts of the world, notably the North West of England. In this film the disease is explored through the eyes of three patients with a range of severity, and different bones involved. The physical findings, diagnosis and investigation (including bone scans) and treatment with modern bisphosphonate drugs are all illustrated.
